New Student Registration Information

Welcome to ATEC!

Below is a list of needed documents for registration. You can also get copies of the school documents from the front office. Call for an appointment at 801-610-8186.

Please print, complete, and bring these forms to the ATEC front desk:
  1. Birth Certificate: We must see an original birth certificate document.
  2. Proof of Residency: Utility bill, rental agreement, closing document, building contract, etc.
  3. Immunization Record: Please bring your most current copy indicating showing requirements are met. (Immunization Requirements)
  4. Any custody documentation: If biological parents are no longer married or there are any issues of guardianship, a copy of the custodial rights, signed by a judge, must be provided at time of enrollment.
  5. Current Copy of Individualized Education Program (IEP)
  6. Guardianship Status
  7. Disclosure Document
  8. Transportation Form
  9. Emergency Contact Form
  10. Permission Form (New Student) (Returning Student)
  11. Sensory Consent Form
  12. Outside Agency Permission
  13. Miscellaneous Forms: Some situations require additional documentation/forms that may be required before registration may be completed.
